On 8/12/25 at 1017 at 1329 I spoke with Andrew, advising him their fire inspection and approval for project under TCO was past due 8/11/25.

In order to avoid citations/fines ALL of the following must be met:
Fire TCO Inspection must be scheduled, and inspector must agree to TCO recommendation.
Ensure all fees pending for this permit or associated permits have been paid.
Send an email request for appointment for required approval to MDFR-FEB@miamidade.gov include your permit number, name, and phone number.

1. Fire TCO not approved.
2. Need to complete the pending items noted under the fire alarm permit.
3. The Sprinklers is partially approved pending revisions and minor items.
4. The access control permit has no inspections yet. Pending.
5. Cannot have greater than a 1/2 inch elevation change at the door threshold. This was found at the ground level of Stairwell 4. Slope it out.
6. All the stairwell signage states NO ROOF ACCESS. This is incorrect. Both stairwells has Roof Access. Remove the NO from the signs on all floors.
7. Seal around the vertical piping penetration with Fire Caulking found inside the FACP room.
8. Provide signage FACP INSIDE at the Door.
9. Provide signage STORAGE ROOM at the door, directly across from the FACP room.
10. Seal around the piping penetrations running through the Storage room. Ground level.
11. The top handrails at the pedestrian ramp needs to return to the wall, floor or itself.
12. Install signage ELEVATOR LOBBY, at the top level.
13. Per NFPA 10, The spacing requirements in parking garages require a maximum travel distance of 30 ft to reach an extinguisher unless the extinguishers are rated a minimum of 40 BC and the travel distance is a minimum of 50 ft. The current Fire extinguishers installed on site are 3A:40B:C, therefore need to provide additional fire extinguishers every 50 feet apart to provide the minimum coverage.
14. Ensure when the security cameras are installed, that only one penetration is going through the stairwells. Per Miami-Dade Fire Policy, we only allow one penetration inside the stairwells for security cameras.
15. All previous pending items from the prior reports was verified and has been completed.

5- RELOCATE FIRE EXTINGUISHERS TO THE OUTSIDE OF THE COLUMNS. FOR VISIBILITY AND ACCESSIBILITY.
6- SOME FLOORS REQUIRED ADDITIONAL EXIT SIGNS. SEVERAL RAMPS DO NOT HAVE ANY. AT THE FLOOR LEVEL THE TWO PROVIDED ARE TOO FAR FROM ONE ANOTHER.
7- PROVIDE THE MAXIMUM HEIGHT CLEARANCE AT BOTH ENTRANCES TO GARAGE AND A HEIGHT CLEARANCE BAR.
8- PROVIDE BOLLARD PROTECTION ON 1ST FLOOR TO PROTECT ELECTRICAL PANELS.
9- PROVIDE A SUPRAMAX BOX AT 1ST FLOOR ELEVATOR LOBBY.
10- FIRE EXTINGUISHERS MISSING FROM 1ST FLOOR EXTINGUISHERS CABINETS.
11- STAIR 3 ON 1ST FLOOR, PROVIDE COVER FOR SPRINKLER RISER J. BOXES.
12- ELECTRICAL CONDUIT FOR SECURITY CAMERAS IN STAIRWELL CAN ONLY BE PENETRATED AT ONE LEVEL.
13- OPENINGS IN BOTH STAIRWELLS AT TOP LEVEL NEED TO BE FILLED AND PATCHED WITH CONCRETE. NOT FIRE CAULKING. FIRE CAULKING NOT DESIGNED TO FILL SUCH LARGE SPACES.
14- SOME LIGHTS IN STAIRWELLS WERE NOT ON. UNKNOWN IF BREAKERS WERE OFF.
15- EMERGENCY AND EXIT LIGHTING PENDING TO BE TESTED ONCE ADDITIONAL EXIT LIGHTS ARE ADDED AND IF TIED TO GENERATOR THEN A DROP TEST SHALL BE PERFORMED.
16- FIRE ALARM, SPRINKLER AND ACCESS CONTROL PERMITS SHALL BE INSPECTED AND APPROVED FOR TCO. ACCESS CONTOL CAN BE DEACTIVATED FOR TCO UNTIL APPROVED.
